• shorts, singlet, bike pants, or other appropriate clothing if required
• your referral (if you have one)
• any scans you may have had
• your private health fund card (if you have one)
• and claim number (if WC or Third party injury)
• list of current medications

Cancellations:

Please advise us if you are unable to attend an appointment, so that we may give that time to another patient if required.

Cancellations need to be made by the previous day or a cancellation fee may be incurred.
